Comprehensive Case Management for Adult Medical Day Services
Empowering Minds Resource Center’s Comprehensive Case Management for Adult Medical Day Services is designed to help participants of the Medical Day Care Services Waiver program access Medicaid and non-Medicaid services. We work closely with you and your family to coordinate services that promote independence, dignity, and quality of life.
What is Comprehensive Case Management?
Comprehensive Case Management is a free service provided through the Maryland Medical Day Care Services Waiver. Your assigned Case Manager works with you to help you understand your options, coordinate services, and ensure your needs are met, allowing you to remain safely in the community.
Case Management services are separate and independent from Adult Medical Day Care centers, ensuring your choices are respected and your services remain conflict-free and participant-directed.
Your Case Manager will:
- Get to know you, your goals, strengths, and needs.
- Help you choose and coordinate services, including Adult Medical Day Care services, Medical and Behavioral health care, Therapy services (PT, OT, Speech, as applicable), Transportation, Durable medical equipment (DME), Community and social supports.
- Monitor your services to make sure they are working.
- Identifying changes or risks early
- Help resolve service gaps or concerns
- Report and address Reportable Events when needed
We provide:
- Regular check-ins by phone or in person
- Prompt response when needs or circumstances change
- Required in-person visits at least every 180 days
