Targeted Case Management for Adults

Empowering Minds aims to provide exceptional support services through Mental Health Targeted Case Management. The Adult Case Management program serves adults who exhibit impairment due to a chronic and persistent mental illness and/or co-occurring substance use disorder. Our qualified team of case managers work with these individuals to assist them with receiving the most appropriate services for recovery and self-sufficiency. Our team is committed to meeting individual needs in the most least-restrictive environment in a timely and efficient manner. 

The primary goal of mental health targeted case management is to connect clients with needed services, including medical, social, educational, mental health, housing, insurance, employment, financial, and other services to support successful functioning in our communities.

Targeted Case Management involves the facilitation of the service planning process to include family and natural support systems; monitoring and evaluating the effectiveness of interventions as evidenced by symptom reduction and progress towards identified goals in the Service Plan; coordination and oversight of initial and ongoing assessment activities; and ensuring linkage to the most clinically appropriate and effective rehabilitative services. Case managers coordinate the provision of quality treatment and support services in the most efficient and helpful way possible, advocating for their clients’ needs, and following up to ensure service effectiveness. The dedicated case manager assists with linkages to federal, state, and/or local resources and provides consumers with the individualized tools they need to reduce the need for a higher level of care.  

These services aim to support an increase in community stability and tenure through referral to behavioral health treatment and support services. Case managers support the attainment of these individualized goals through the coordination of services which include but are not limited to mental health counseling, psychosocial rehabilitation, primary care and medical specialists, crisis services, social security administration, legal services, vocational rehabilitation, educational and other services.

Case Managers work closely with the client and their support team to assist in navigating mental health care, while advocating for their needs across domains of functioning. Our method of personalized Targeted Case Management offers a tailored and individual approach that can be modified as necessary to accommodate the specific needs of every client.

Overall, our goal is to ensure that all qualified individuals have someone to advocate for their needs and help them find resources within their communities that will promote healthy living, optimal individual functioning, and resiliency.
