Psychiatric Rehabilitation Program (PRP) for Minors

The Psychiatric Rehabilitation Program (PRP) at Empowering Minds is designed to promote positive development of a youth's behavior, social skills and family relationships. The program is person-centered and therefore strives for the empowerment of the consumer, which leads to the attainment of personal goals and objectives.

PRP services are provided in the setting most conducive to enhancing each youth's own healthy participation in the community. This may include the youth's home, foster care home, job site, educational setting or other community sites. The intensity of services will vary depending upon the youth's individual needs and/or environment.

Psychiatric Rehabilitation Program (PRP) for Adults

Empowering Minds Psychiatric Rehabilitation Program for Adults provide individuals with a psychiatric disability (See Priority Population Diagnosis List) the opportunity to work, improve necessary interpersonal skills, live in the community, and enjoy a social life, at their own pace, through planning experiences in a respectful, supportive manner. Our goal is to help the client reach independence in the community with minimal professional support. Our activities are provided both offsite and on-site with the support of our trained Family Service Coordinators.

Comprehensive Case Management for Adult Medical Day Care Services

Comprehensive Case Management for Adult Medical Day Care Services is designed to help participants of the Medical Day Care Services Waiver program access Medicaid and non-Medicaid services. We work closely with you and your family to coordinate services that promote independence, dignity, and quality of life.
